Effective March 6th, 2023 This Privacy Policy describes the information collection and privacy practices of Jacquelyn Levin DO Inc. (hereafter, "we," "our," or "us") on the skinknowing.com website (hereafter, the "Site"). We respect your privacy and are committed to the privacy and confidentiality of your personal information. This policy describes what personally identifiable information is collected from you through the Site, how it is used, with whom it may be shared, and how you may update the information or opt out. Information Collection As with other Internet services, our Site gathers information automatically each time your browser or device interacts with the Site, and store that information in "log files." The information stored in our web servers' log files includes Usage Data and Tracking Data, as described more fully below. As used herein: • "Cookies are small files that contain a small amount of data, typically including a unique identifier. They are generally placed on your browser by a website or other browser-based service. Our Site uses cookies as more fully described herein. • "Medical Information" shall have the meaning provided by applicable law and may include medical records, symptoms, examination and test results, diagnoses, treatments and plans for future care or treatment. • "Personal Information" is information about an individual that can be used to identify that individual and may include names, addresses, Cookies, device identifiers, precise location data and biometric data as provided by applicable law. • "Tracking Data" is information collected by Cookies, pixel tags and other tracking technologies and information that your browser sends to our servers when you visit the Site. You do not have to give us Personal Information to visit the Site and we collect no Personal Information about you when you visit our Site unless you choose to provide that information to us. We do not disclose, give, sell, or transfer any Personal Information about our visitors unless required by law enforcement or statute. We may aggregate or de-identify information collected by the Site. The Site uses session-based cookies and stores information about the selections a user has made within the system. This information is stored on the server and associated with an id number. The information is stored so that a returning user can restore their selections. No information is stored on the user's browser or elsewhere that could be used to associate an individual with a set of stored selections. Use of Medical Information We are required by law to maintain the privacy of user Medical Information, to provide users with a notice of our legal duties and privacy practices, and to follow the information practices that are described in this notice. This notice explains how your Medical Information may be used and/or disclosed and also describes the rights you have concerning your own Medical Information. Your Medical Information will not be used or disclosed except as indicated in this notice. You have a right to request and receive a paper copy of this notice. The Site does not create user medical records and no Medical Information is associated with your user profile, Personal Information or other information or data that can be used to determine the identity of the user providing such Medical Information. Any Medical Information you choose to provide, such as in connection with a survey, will be anonymized and aggregated and, once provided, cannot be de-anonymized, de-aggregated or retrieved. You are prohibited from using the Site's user-generated content features (e.g., comments, message boards) to disclose your Medical Information and we disclaim any responsibility, obligation or liability arising from such public disclosure to the fullest extent allowable under applicable law. We may use or disclose aggregated and anonymized Medical Information for any purpose. These third parties have access to your Personal Information only to perform these tasks on our behalf. Personal Information may be disclosed to entities we control, by which we are controlled or with which we are under common control (affiliates) for purposes consistent with this Privacy Policy or with a third party to which we have transferred our business, assets or stock if we are involved in a reorganization, merger, sale, joint venture, assignment, transfer or other disposition of some or all of our business, assets or stock, including during due diligence related to any such transaction (and also including in connection with any bankruptcy or similar proceeding). The Site may provide functionality that enables you to disclose information, including Personal Information, to other users or the public. For instance, you may be able to maintain a user profile with information about yourself that you can make available to other users or the public. You may also be able to submit content (such as comments, reviews, surveys, blogs, photos, and videos), and we may display your name, username, and a link to your user profile along with the content you submit. We do not control how other users or third parties use any Personal Information that you make available to other users or the public. Once you have posted information publicly, while you will still be able to edit and delete it from the Site, you will not be able to edit or delete information cached, collected, and stored elsewhere by others (e.g., search engines). Please do not disclose any sensitive Personal Information (e.g., social security number, detailed financial information, information related to racial or ethnic origin, political opinions, religion or other beliefs, criminal background or trade union membership). If you have requested that some Personal Information be deleted, note that we may need to retain some of the information for backup or compliance purposes. In addition, there could remain residual traces of the deleted Personal Information in our databases and other records. We are not responsible for changing or deleting information from the databases of third parties that have previously accessed your information. You may opt out from receiving marketing-related messages from us either by using the unsubscribe mechanism provided in the message or by emailing us at contactus@skinknowing.com. If you opt out from receiving marketing-related messages from us, we may still send administrative messages to you. You cannot opt out from receiving administrative messages. You have the right to lodge a complaint with a supervisory authority as to our processing of data. However, we hope that you will first consult with us, by emailing us at contactus@skinknowing.com so that we may work with you to resolve any complaint or concern you might have. The Site does not support the Do Not Track browser setting. Use of Tracking Data We use Tracking Data (including third party tracking tools) to gather statistics on the traffic to our Site, evaluate user interest in various areas of our Site, to help us make our Site more useful to visitors and to tailor Site content and advertising to your interests. We may use and disclose Tracking Data collected on the Site for any purpose permitted by applicable law. Note: If we are required by law to treat any element of Tracking Data as Personal Information, then we will handle those elements as Personal Information. If we combine any Tracking Data with Personal Information, we will treat the combined information as Personal Information as long as it is combined. Transfer of Information We may need to transfer your Personal Information outside of the country from which it was originally provided. Changes to this Privacy Policy We may update this Privacy Policy to reflect changes to our practices. We will post all Privacy Policy changes on this page and, if the changes are significant, we will provide a more prominent notice. The Privacy Policy posted on this page will say when that version went into effect. If you visit the Site after a change, then you will have accepted the new Privacy Policy.
